Home Health Care Services

Public Health Services have been a cornerstone of Indian Health Services and Tribal Health Services with a mission to provide health services and health education to improve the health and wellbeing of the Native population. This model application is typically a broad stroke and not individualized. As a result, many agencies have attempted to create Home Health Programs from existing Public Health Programs to meet the specific health support needs of individuals in their service area.

Older Americans are living with five or more chronic conditions. These rates are much higher in the Native American population. Effective care management of this population is often complicated by several other health and social risk factors. Unfortunately, multiple studies reveal that the health care needs of older adults are poorly managed, often with devastating human and economic consequences. Native Americans coping with multiple chronic conditions and complex therapies are particularly vulnerable to breakdowns in care.

North American Tribal Organization has clinical personnel who have extensive experience in developing and implementing home health programs that are Case Management/Utilization Management focused to provide not only direct services in the home but a team approach to providing care outside of the facility to improve the health of the identified population and reduce re-admission and inappropriate utilization. Along with these benefits, North American Tribal Organization can assist in the CMS certification process of Home Health Care programs to maximize reimbursement.
